If you or your group is interested in using one of the Ames Community School District's facilities, please complete the Facility Usage Form below.

Please complete ALL pages of the Facilities Usage Form. This form is in Microsoft Word format and can be edited directly by those using Microsoft Word, or printed and filled out as needed by downloading the form below.

Once the form has been completed you may return it by:

1. E-mailing it back to us
( * please change the file name and indicate the new name in the email - save file as "yourname.doc" not "FacilitiesUsage.doc" )
after filling the form out and saving it on your computer.
